"49cc Scooter Carburetor GY6 Four Stroke with Jet Upgrades by NIMTEK" LinkThis carb has an acceleration pump. Are you giving the throttle a twist, then energizing the starter while throttle is CLOSED so max vacuum pulls fuel through the low-speed pathway? Perhaps water in gas is stratifying at bottom of carb bowl. Consider adding around 3 tablespoons of Iso-Heet to the tank when refilling.

You have a lot of good info here maybe i can add ! Air fuel screw how many turns is it out ? the further out the bigger the issue1 LOL at what point when your going out does it stop affecting the idle ? How many turns ? Did you check the Manifold for vacuum leaks at the base of a crack or the spacer ? did it ever start cold properly? Is the enricher valve tight ? Unplug it and attempt to start it what happens cold ? Did you inspect inside the Bowl of the new carb before you installed it? I know this sounds like a lot of work and it is so don't give up on it ! lol O yeah are you give it a quick pump of the throttle be for you fire it up? Some scooters need it some don't all depends on your carb quality ,Or did you get a low quality carb and not a Genuine CVK brand
