I've had multiple kick starts levers slide off and abandon ship while I'm riding. I've tightened that sucker as far as it'll go. The shaft is aftermarket and the lever is "universal" so that may have something to do with it. Is there anything I can do to make it more secure?

If the threads and the splines are in good shape and match as they should, it should stay with just bolting it on. Replace damaged, worn or mismatched parts first if possible and see if that solves it.
I've ground the gap in the lever slightly larger before to allow it to clamp down tighter. Also used a long bolt with a nut as insurance.

Isn't there a groove in the middle of the splines for the pinch bolt to slide in? On a Zuma the bolt has to be removed to slide the lever off. Sounds like 90GTVert said, the lever and shaft are mismatched.
